• No Show Fee 

Please note that even after completing the Pre-Registration form, you are still required to complete the Health Questionnaire, Policies, and Agreement forms.

Please bring a list of your current medications, all test results  pertaining to your visit including  Xray reports, CT Scans, MRIs, and any Lab results at the time of your appointment.
